Description
Darlene is entering her senior year, and will be receiving a special high school diploma. She has a history of learning disabilities and ADHD. Darlene currently lives with her mother but is very independent and wishes to move out after she graduates. Darlene wishes to drive in some part of the transportation industry.

Employment Goal: Darlene will work in the transportation industry.
Level of performance: Darlene has had limited work experience. She has volunteered at her church. She does have a disability, but brings a good attitude towards working.
Steps needed to accomplish goal: 1. Obtain vocational training.
2. Provide social interaction training.
3. Obtain supported employment services.
*Darlene took a career inventory questionnaire; which did not reflect her desire to work in
the transportation field.

Vocational Education/Training Goal: Darlene will ride along with carious types of transportation providers. This will help her determine which branch of transportation she might enjoy working in.
Level of present performance: Darlene has no experience in this field, but she has a driver's license. She currently commutes to and from school and other facilities.
Steps needed to accomplish goal: 1. Arrange for Darlene to ride along with a local taxi driver.
2. Arrange for Darlene to observe a pilot transporting passengers on an airplane.
3. Arrange for Darlene to ride along with a local bus service.

Postsecondary Education Goal: Darlene will attend the local community college to take Personal Enrichment Courses.
Level of Present Performance: Despite Darlene's disabilities she has done very well academically and has a very positive attitude toward learning.
Steps Needed to Accomplish Goal: 1. Arrange for Darlene to have a tour of the community college.
2. Arrange for Darlene to meet with college advisor.
3. Assist Darlene in filling out the admissions application in order to start the enrollment process.

Financial/Income Needs Goal: Darlene will have her own bank account for allowance money and future income.
Level of present performance: Darlene has not means of saving money at the present time. The allowance money that she receives is put in a jar. However, she is easily tempted to spend it rather then saving her money.
Steps needed to accomplish goal: 1. Arrange for Darlene to have lessons in money management.
2. Arrange for someone to oversee Darlene's finances for a preliminary 3-month period.
3. Arrange for Darlene to visit several banks to decide where she wants to bank.

Level of Present Performance: Darlene knows how to take care of herself, she has lived independently most of her life with the absence of her mother.

2. Arrange for Darlene to see different kinds of living facilities.
3. Discuss with Darlene the option of living in an assisted facility or independently.

Transportation/Mobility Goal: This goal has already been accomplished. Darlene already has a driver's license and is a functional driver.

Level of Present performance: Darlene is usually on best behavior but occasionally acts out without her medication.
Steps needed to accomplish goal: 1. Provide a watch with an alarm to remind Darlene to take medication.

3. Join pep club to be more involved in social activities with her high school.

Self-Advocacy/Future Planning: Darlene will benefit from moving into a group home while in high school.
Level of present performance: Darlene is currently living with her mother who is not always around. Therefore it would be in her best interest to move into a group home, until she is of age to live independently.
Steps needed to accomplish goal: 1.Visit and evaluate local group homes.
2. Choose group home that best fits needs.

Darlene's Case
Darlene will be entering her final year of school in the fall. She has a long history of
learning disability and attention-deficit/hyperactivity disorder. According to her guidance counselor, Darlene
will be receiving a special high school diploma.
Darlene's mother has been supportive for the past 2 years; however, she has been absent many times before. As a result, Darlene is very independent and is determined to be as successful as possible. She is interested in working, perhaps in the transportation industry, as a driver. Darlene has a good attitude toward work and brings many assets to vocational and transition planning. Despite discouraging words from family and friends about her chosen career, Darlene is determined to succeed.
